Since 1972, Foothills Gateway, Inc. has improved the lives of thousands of people in northern Colorado. Originally founded to provide case management and services specifically for people with intellectual and developmental disabilities (IDD). Today, Foothills Gateway, Inc. is the Case Management Agency for Larimer County, primarily providing assistance to people and families accessing state and federally funded programs, such as Medicaid Waivers. Foothills Gateway, Inc. is an organization that believes in listening to people. We believe in building respectful and trusting relationships between the individuals and families we support, as well as our employees, volunteers, and agency leaders.
Case Management services include: Early Intervention Service Coordination, Intake and Enrollment, Long Term Services and Supports for both adults and children, state-funded Supported Living Services, PACE and Skilled Nursing Facility admissions, Family Support Services, and Housing Choice vouchers.
Direct services offered include: Transportation, Job Development and Placement Services, Adult Care Services (respite), Early Intervention services, Children's Extensive Support services, and behavioral and crisis response services.
