Since 1972, Foothills Gateway, Inc. has improved the lives of thousands of people in northern Colorado. Originally founded to provide case management and services specifically for people with intellectual and developmental disabilities (IDD). Today, Foothills Gateway, Inc. is the Case Management Agency for Larimer County, primarily providing assistance to people and families accessing state and federally funded programs, such as Medicaid Waivers. Foothills Gateway, Inc. is an organization that believes in listening to people. We believe in building respectful and trusting relationships between the individuals and families we support, as well as our employees, volunteers, and agency leaders.
Case Management services include: Early Intervention Service Coordination, Intake and Enrollment, Long Term Services and Supports for both adults and children, state-funded Supported Living Services, PACE and Skilled Nursing Facility admissions, Family Support Services, and Housing Choice vouchers.
Direct services offered include: Transportation, Job Development and Placement Services, Adult Care Services (respite), Early Intervention services, Children's Extensive Support services, and behavioral and crisis response services.

Foothills Gateway, Inc. (FGI) was established in 1972 though a merger of the Foothills Activity Center in Loveland and the Easter Seals Gateway Center in Fort Collins. Originally named the Foothills Gateway Rehabilitation Center, services at that time were provided to 65 individuals with intellectual and developmental disabilities in the areas of education, occupational therapy, speech therapy, social services, and a sheltered workshop.
Renamed Foothills Gateway, Inc, the agency is now the Case Management Agency for Larimer County providing case management services to over 4,500 individuals in northern Colorado. In addition, FGI provides select services to individuals with intellectual and developmental disabilities in Larimer County.
