Hospice of Montezuma is a community non-profit organization of dedicated professionals and volunteers providing in-home assistance and guidance to all individuals and families.

Background Statement

In 1989, Hospice of Montezuma was incorporated and built a reputation for excellence in interdisciplinary care. In 1999, it was recognized as Hospice of the Year by the Colorado Hospice Organization. It has worked with community agencies to provide an efficient referral network to meet diverse needs of individuals. Hospice of Montezuma moved to its 512 N. Broadway in Cortez, CO in 2016. The greater visibility and easy access have afforded staff to address the growing needs of the community. Hospice care is offered in area nursing homes, assisted living facilities, the local hospital, and in patient/family homes. Counseling sessions are offered within the agency, individual homes, and convenient community spaces. Hospice of Montezuma has partnered with the National Hospice and Palliative Care Organization's "We Honor Veterans Program" to support post acute needs in healthcare and rehabilitation. It has a history of honoring its mission statement of being a community non-profit organization of dedicated professionals and volunteers providing in-home assistance and guidance to individuals and families.

Hospice of Montezuma employs 20 professionals that offer consultation and public counseling. It services individuals living in their rural homes. Both Montezuma and Dolores Counties are part of our service area in rural southwest Colorado. Dolores County is one of the few remaining Frontier Regions (defined by less than four individuals per square mile). These counties have limited assisted living and nursing home facilities and do not have public transportation with regularly scheduled routes. It is important for individuals to maintain their autonomy, a concept supported by Hospice of Montezuma. The combined counties have approximately 27,500 residents. In addition to hospice services, our agency hosts group bereavement, caregiver, suicide support classes and educational opportunities.
