Mission

We foster partnerships so we can bridge the gap in care and provide access to quality resources within our community.​

Many people think about genetics and medical care when they think about their health. But studies show that 60% of what determines our overall health is attributable to social, environmental, and behavioral factors. Because healthcare and social services operate independently in silos, it can be difficult to address these needs that are imperative in improving the health and lives of individuals.​

THP leads the way in bridging the divide, enabling healthcare agencies to work with community organizations to mitigate social determinants of health. The needs of each community are complex and require a whole person approach to develop sustainable solutions. The Health Partnership helps its partners align and strengthen their efforts to achieve the outcomes most critical to the individuals and communities we serve.​

The places and conditions where people work, live, and play matter - they meaningfully shape health and life outcomes. By implementing solutions that address health disparities, we can bridge the gap in care and provide access to quality resources to our community.​

Background Statement

This agenda became the foundation for Northwest Colorado Community Health Partnership, which in 2019 became The Health Partnership. We believe partnering with community organizations, and communicating consistently and transparently, can help individuals and community service partners overcome barriers to health and well-being.

The Partnership has been providing structure and fiscal support for this collective care model with backing from the Regional Accountable Entity (RAE), Rocky Mountain Health Plans, partnership dues and grants. We are a 501(c)(3) nonprofit organization and became fiscally independent from Northwest Colorado Health in 2016. With the changing health care landscape moving from episodic care to focusing on whole person wellness, The Health Partnership is poised to ensure people have access to and are connected with the services, programs and support they need to thrive.

In 2017, we launched Navigation Networks to bring together our workforce (case managers, navigators, care coordinators) to work collaboratively to ensure people in our communities were connected to the right resources at the right time. Through this effort, we have compiled county-specific resource and referral lists for our network and clinical partners, available in English and Spanish. The resource and referral lists are updated regularly and have become increasingly valuable to our community partners as we work together to support people in need.

In 2018, the Navigation Networks recognized the need for collaborative care meetings. Since then, we have been hosting collaborative care meetings to ensure wraparound services for our clients. These care team meetings allow those in the navigation role to work more efficiently and effectively while supporting the same clients. In late 2020, we aim to bring the Community Resource Network online to be able to provide virtual care team collaboration meetings.

Throughout our history, we have continued to provide quality community-based care coordination services to more than 500 people annually. From our clients we learn about the structural barriers our community impact team works to address.
