Northwest Colorado Health was founded in Steamboat Springs, Colorado in 1964. What started out as a small agency providing only public health services and home health care has grown to also provide primary care, hospice care, and wellness and prevention services to all members of our community. With our programs, our facilities have grown, as well.

1964 Established as a Public Health and Home Health Agency

1995 Merged with Hospice of Steamboat Springs

2005 Acquired The Haven Assisted Living

2007 Completed Construction of The Haven Community Center

2008 Opened the Community Health Center in Moffat County

2009 Opened the Rollingstone Respite House in Routt County

2013 Opened the Community Health Center in Routt County

2016 Began providing Home Health and Hospice and Palliative Care services in Grand County

2017 Opened Dental Clinic in Moffat County

2018 Began providing dental services at the South Routt Medical Center in Routt County

2021 Began providing School-Based Health Dental Services in Routt and Moffat County

2022 Expanded our Community Health Center in Steamboat Springs to include a full Dental Clinic

2024 Acquired Casey's Pond Senior Living Community in Steamboat Springs

Since our inception more almost 60 years ago, Northwest Colorado Health has embraced continuous performance improvement in order to provide all residents of Northwest Colorado with the topnotch, timely health care services that they need. Northwest Colorado Health continues to innovate and keep pace with healthcare best practices and national health care reform, such as receiving Patient Centered Medical Home recognition, leading a regional integrated behavioral health program and expanding insurance enrollment services to reach four counties in Northwest Colorado.

Northwest Colorado Health provides health programming across the region to people of all ages, cultures, income levels and life situations. All services are offered on a sliding scale based on family size and income. Northwest Colorado Health gives all residents of Northwest Colorado a chance to live a healthy life through five major program areas: Prevention Services, Home Health, Hospice, Primary/Dental/Behavioral Health Care and The Haven Assisted Living.
